How To Complete Your SDDC Survey

If your Government Bill of Lading (GBL) number looks like this ZX-123456, follow these instructions:

1. If you have already received your password from your origin Transportation Office, proceed to step 2. If not
   1. Go to the website: https://icss.eta.sddc.army.mil and click on the "Forgot Password" link. Enter the required information.
   2. You will receive a confirmation that your request has been received and a password will be sent to the email address on file.
2. Once you have your password, go the following website: https://icss.eta.sddc.army.mil and enter your Social Security Number (USCG use Employee ID) and password and click on the submit button.
   1. Please note that the Transportation Office has to enter your delivery information into their system, so it is possible you will receive a message saying there are no surveys available for you until they do.
3. Once you are logged in you may complete the survey.
4. If you experience any trouble completing your survey you may call (800) 462-2176 and choose option 7.

If you moved under the DP3 Program your GBL number looks like this ABCD0000123, follow these instructions:

1. If you have a DPS User ID and Password, proceed to step 2. If not,
   1. Go to the following website: http://www.move.mil.
   2. Click on DOD Customer at the top of the screen.
   3. Click on "Register DPS Account".
   4. Complete the requested information and click “Submit”.
2. Once you receive your User ID and Password:
   1. Go to the following website: http://www.move.mil.
   2. Click on "Login To DPS" (First time users must click Service-Specific link).
   3. Click the "Customer Satisfaction Survey" tab at the top of the screen.
   4. Complete the survey.
